How do I Obtain Services?A registered nurse visits the home to assess the patient's care needs and determines the family's ability to participate in care. A care plan is then established based on the nurse's observations, family needs, and physician's orders. A registered nurse at Tri County Home Health is always available to answer questions about home health care and is able to start care within a few hours of notice. Tri County Home Health staff will contact your physician for further information.
