How do I get a Physician Referral?

Tri County Home Health offers you a fast and easy method for home health consultation and admission.  

  1. You can print our referral form, complete it, and FAX to Tri County Home Heath at 830-895-3102 .

  2. You can download the form, complete it online, and email to tchhadm@ktc.com as an attachment.

Once the FAX or email is received, our Case Manager RN will contact your office to complete the referral and admission process.
